Hearing the stories of hundreds of these displaced persons, one theme resonated over and over – lack of money for chronic care management. Most of these individuals are suffering from conditions like hypertension, diabetes, and hyperlipidemia and they have to make a choice between getting food and buying medication. So staying on top of their medication regimen and routine check-ups is not feasible for them. CRRI plans to provide a continuous supply of these medications to these persons, so there is no lapse in treatment. Because some of these individuals do not even know their medication regimen, CRRI plans to provide basic screening by some health practitioners so they can be placed on a medication regimen that CRRI can sustain long term.
